Working with a group of talented designers and content editors, DB Design implemented a graphic mock-up of the approved design using the latest in XHTML/CSS web development.

Electronic Forms Project

DB Design was contracted to create a digital capture system of some of their forms used on the van. The challenge: capture the data on tablet-based computers via a broadband cellular service while maintaining a secure, robust, uninterrupted internet connection to the database server where the data were to be stored at Harvard Medical School's data center. Also, a primary goal was to create 'smart forms' where information can be entered and the form can adapt to the clients answers appropriately. Some of the resulting features include:

Auto-Save feature saves form every minute through an asynchronous submittal process (AJAX), completely unobtrusive to the staff filling it out.

Tablet-friendly input of data, through dynamic number pads, date-pickers, and the like.

Completely anonymous storage and retrieval of demographic information about repeat clients via AJAX.

Context-Sensitive Question Generation removes and adds questions based on responses to previous questions. An example is if the client is male the form will not include pregnancy questions.

Auto-Prompt of important information based on previous questions and calculations. An example is if the client's blood pressure is high, the form will remind the staff to ask about hypertension.

These forms promise to streamline the information gathering process and ensure good, useful data are stored in an optimized central database for reporting.
